When you enroll in the New York State Donate Life Registry, you give your legal consent for the recovery of your organs, eyes, and tissues for the purposes of transplantation and research at the time of your death.

You have the option of specifying what organs and tissues you wish to donate; you may also choose to donate for transplantation or research. You must be at least 18 years of age to register.

The Donate Life Registry information is administered by the New York State Department of Health. The registry is a confidential database. It is only accessed by federally regulated organ procurement organizations, such as the New York Organ Donor Network, and New York State licensed eye and tissue banks to identify potential donors at or near the time of death.
